[JUL-19-22 Rance Kilbourn] Walked job and reviewed field AF label and panel schedule. Both match FAC Engineering records. Contacted RI and discussed the job scope. Advised to edit scope to include only work to be performed after the LOTO is established. Placed on RI hold corrections needed.

[JUL-20-22 William Michael Spier] On July 19, 22 Rance Kilbourn asked me to update the description to just what was involved with the work scope, I provided too much information. I updated the description 7-20-2022

[JUL-20-22 Rance Kilbourn] Reviewed changes to the job scope. The information is clear and accurate. Updated with adding a Mandatory Special Instruction regarding the ZVV process. After speaking with the ESO he identified an opportunity to add some efficiency to the LOTO ZVV procedure. He concluded that when working on multiple points within a single electrical circuit, only the nearest point to the power source needs to be evaluated by the FAC electricians with a Zero Voltage Verification. The contractor will then be responsible for doing a First Exposure at each point there after along the circuit.

LP-4656 07-06-2022 HEC 1 (480V 1) Cereno, Fred Crouse Construction B50B Boiler flue Replacement 50B Demo and remove existing flue stack and exhaust fan.

To replace the existing flue pipe for BR-003-50B, BR-004-50B and BR-005-50B with new flue pipe and flue draft controls.

The following work is included:
- Demo the existing flue in the mechanical room;
- Demo the existing flue fan on the roof;
- Install new flue liner in the existing flue chimney;
- Install new double wall flue in the mechanical room;
- Install new modulating flue fan on the roof;

[JUN-27-22 Teilhard Valladares] Work on the flue stack and exhaust fan on the roof will require boilers 3 and 4 to be shutdown and LOTO'd to prevent accidental start-up. CO2 levels are too high when the boilers are running at the flue stacks.

E-mailed Engineering and EHS for recommendation for switching the breakers feeding the boilers. Panel  38A11A is an overdutied panel.

[JUN-27-22 Teilhard Valladares] Will need a detailed scope on the demo of the flue stack and exhaust blower. Contacted RI for scope update.

[JUN-28-22 Teilhard Valladares] Updated and added 50 BR 003 and 50 BR 004 to the electrical isolations.

[JUN-29-22 Teilhard Valladares] Added mandatory instructions. Included switching controls for PNL 38A11A (overdutied panel) per Engineering and EHS recommendations.

[JUN-29-22 Teilhard Valladares] Uploaded field arc flash labels and EHS report on the CO2 levels at the roof flue stack.

Waiting on the complete and detailed scope of work before going forward with the permit.

[JUL-06-22 Teilhard Valladares] See below for switching recommendations for PNL 3811A from Engineering and DESO.

Panel 38A11A:
AF: 0.94 cal/cm2.
Available 3 phase short circuit: 39kA.
The panel is deemed to be overdutied because of the breaker installed in slot 6,9 and 22. Breakers 2 and 4 have the appropriate KAIN rating of 65kAIC.

"I recommend to perform switching on breaker 2 and 4 with the appropriate PPE based on the voltage and incident energy. "

"Approved as described. No need for the 40 Cal suit in this case if you are not switching the under-rated circuit breakers."

[JUL-06-22 Fred Cereno] Added to SOW: To replace the existing flue pipe for BR-003-50B, BR-004-50B and BR-005-50B with new flue pipe and flue draft controls.

[JUL-13-22 Teilhard Valladares] Field walked 7/12 with RI and subcontractors. Verified scope of work with RI and subcontractors. Per conversation with RI and subcontractors, 2 additional LOTO permits will be submitted. One for replacing the circuit breakers at panel 38A11A and running new circuit for the control panel and flue dampers. The second permit will be for Boiler 5, boiler 5 share the same exhaust ducting as boiler 3 and 4. Boiler 5 is the domestic hot water boiler that serves the building.  RI and engineering are planning to temporarily re-route the exhaust ducting for boiler 5 so building can continue to have domestic hot water.

[JUL-13-22 Teilhard Valladares] If modification is needed for the conductors to the new blower motor on the roof. LOTO isolation point will move from the switch to the breaker. This will affect FPU-2A-50B/ Microstar per panel schedule.

[JUL-14-22 mark liljefelt] Field verified isolation point. Note minor AF discrepancy between field label and FAC records for PNL38A11A. AF PPE levels are not affected.

When you bring additional fields into a conversion, Quickbase often finds inconsistencies. For example, say you're converting your Companies column into its own table. One company, Acme Corporation, has offices in New York, Dallas and Portland. So, when you add the City column to the conversion, Quickbase finds three different locations for Acme. A single value in the column you're converting can only match one value in any additional field. Quickbase needs you to clean up the extra cities before it can create your new table. To do so, you have one of two choices:

  • If you want to create three separate Acme records (Acme-New York, Acme-Dallas and Acme-Portland) click the Conform link at the top of the column.
  • If the dissimilar entries are mistakes (say Acme only has one office in New York and the other locations are data-entry errors) go back into your table and correct the inconsistencies—in this case, changing all locations to New York. Then try the conversion again.
